2013-14 Athletic Physical Packet
Any student participating in a summer camp and or sport for the school year must complete an Athletic Packet and be cleared by the Athletic office before participation.
Please read and complete the packet below -
Return pages 8 - 12 to Athletic Office to be approved.
You must be approved before any try-out or practice. Physicals must be dated after April 1 and contain a doctor's stamp.
