Want to participate in a sport next year?  These steps must be done BEFORE tryouts, practices or summer camps.

1.  Download the athletic packet.

2.  Fill out forms and sign.

3.  Take the back page to a doctor for a sports physical. Make sure it is signed by the doctor and stamped with doctor info.

4.  Bring completed packet to the athletic office.

5.  Mrs. Treto will hand you a clearance form.

6.  Take clearance form to coach on day of tryout/practice/camp.
